• Immunization Records
• Primary Proof of Residence (One Required)
  • Home mortgage statement
  • Builder’s agreement
  • Purchase agreement
  • Homeowners insurance policy
  • Property tax statement
  • Lease/rental agreement
• Secondary Proof of Residence (One Required)
  • Current gas, electricity, or water bill
  • Current phone, cable, or Direct TV bill
  • Department of Social Services documentation
